Scientific evidence supports the effectiveness of community-led health initiatives and decentralized health systems. However, these findings are often not integrated into global health policy due to institutional inertia and funding constraints.
The Seventy-ninth World Health Assembly reflects a global health system in need of transformation.
Indigenous knowledge, historical awareness, and cross-cultural insights reveal the limitations of current governance models, which are shaped by colonial legacies and power imbalances. Scientific evidence supports decentralized, community-led health systems, yet these are often sidelined in favor of top-down, Western-centric approaches. To achieve health equity, future health governance must be restructured to include marginalized voices, integrate diverse health knowledge systems, and reform funding and trade policies. This requires a systemic shift that acknowledges the interconnectedness of health, power, and culture, and prioritizes justice over institutional inertia.
